Horizon Support Services, Inc. provides Supported Living Services for individuals who want to live as independently as possible in a home of their choice. Unlike a traditional group home setting, Supported Living is centered around the participant’s own home or apartment, with individualized assistance provided according to their needs, preferences, goals, and Person-Centered Plan.
Our trained Direct Support Professionals work alongside each participant to build the skills and confidence needed for everyday life. Depending on the individual’s needs, support may include personal care, meal preparation, household responsibilities, self-care, problem-solving, transportation, community activities, and other activities of daily living. Delegated nursing support may also be provided when identified as an assessed need.
At Horizon, supported living is about more than receiving assistance. It is about helping each person have greater choice and control over their own life. Participants are supported in making everyday decisions, developing greater self-direction, participating in activities they enjoy, building relationships, and becoming more involved in their communities.
Our goal is simple: to help each participant live safely, confidently, and as independently as possible while creating a life that truly feels like their own.
